We have the exact same problem. We just moved into a new house and every once in a while we notice these little black worms about 1/8" on the floor of the shower stall floor after we shower. I noticed that they hid in the cracks where the grout had come apart. So we had the entire shower stall re-grouted and the corners and fixtures siliconed. This did not work, they still appeared. So I followed one crawling (slowly) near the shower door. Knowing that the shower door frame is hollow aluminum, I thought the nest (or whatever you would call it) could be inside. So we had the shower door replaced. They still appeared! I must've poured every type of drain cleaner and bleach down the drain including "Dran-O FOAMING" hoping to kill them. They STILL occasionally appear. My next step is to replace all the drainage piping which would require the plumbers to remove the ceiling in our first floor bathroom (a major undertaking). I do not know what else to do, my wife is completely disgusted and does not want to shower in our home. Please let me know if you discover anything that helps. Thanks.
