could these be skin parasites?

I went to my Dr.'s awhile back because my hands were very scaly and itchy. (feet too) She said it was nothing. Ok now when I use hand sanitizer little black things come off. It has gotten worse. At first look they look ok, but is I scrape them gross stuff comes off. Any ideas?
